Please fill out the following form below to request the LHCC van for ministry use.
Final approval will be at the discretion of the Transportation lead. A follow up email will be sent out upon completion of this form. Thank you!
To the Ministry Lead & Driver:
It is your responsibility to keep the van and the people or cargo inside it in proper condition. Please observe all traffic laws and roads with extra caution (this includes seat belts for all passengers, speed bumps, railroad crossings, etc.), especially when driving in mountainous terrain. Please observe the speed limit at all times (DO NOT exceed 65 mph on highways).
Upon return please make sure that you park the van in the back parking lot and remove any trash or personal belongings so that the next group will have a clean van to use. If the gas is less than a 1/4 tank please fill it up and submit a CRV along with the receipt and this form.
Governor Jerry Brown signed AB 1785 into effect last year, and begins now in 2017, and it prohibits holding a cellphone while driving. It doesn’t matter why you’re holding the phone – if you’re driving and holding it, you’re breaking the law. Breaking the new law could subject you to a fine of $20 for the first offense, and fines increase to $50 with subsequent infractions. Please drive safely!
